If you do some research online you will find a lot (and I mean a lot) of different resources and tools to show you how to create a personal development plan. Most of which are good (really good) sources of knowledge but they almost always contain a lot of really wordy text, a few fancy bubble diagrams and often come from a white haired old guy in a suit... oh and they want you to buy something from them. Please don’t buy into this. You are already going off track before you have started. A PDP is about personal growth, not getting somebody else to tell you how to do it. It’s about finding what’s important to you, understanding these as your core values, setting goals that you want to achieve, setting end dates and reviewing your progress when you get there! EASY!
